The practice reserves the right to refuse to accept a patient, for example:

  • Where the personal safety of practice staff may be compromised;

Carers and the Cared For

When registering with the practice, you will be asked if you are a carer or are cared for. As a carer you and the person you care for will be asked to complete the appropriate consent form to enable us to hold appropriate information in your medical record.

Similarly, if you are cared for, you will need to ask your carer’s permission to hold such information in your medical record.
