OBTAIN A REPEAT PRESCRIPTION?
Request slips for routine medications
may be issued by your doctor and can be used to order further prescriptions. Please
allow 24 hours for the request to be processed, and if you wish this to be returned
to you by post, please enclose a stamped addressed envelope. Most chemists in
the Ryde area operate a prescription collection service - ask at reception or
your local pharmacy for details.
In order to avoid errors WE DO NOT
ACCEPT REQUESTS FOR REPEAT PRESCRIPTIONS BY TELEPHONE OR WITHOUT A REQUEST SLIP.

OBTAIN MEDICAL CERTIFICATES?
Periods of absence from work of six
days or less do not need a doctor's certificate. Self-certificates for the first
week of illness are available at the reception desk.
