NOTIFY CHANGES TO MY PERSONAL DETAILS?
If you move house or change your
name please let the surgery know as soon as possible. If you move outside our
practice area, you may need to register with a surgery in your new locale. Our receptionists will be happy to assist you with this.
Patients' Telephone Numbers
It would help us a great deal if
you gave us your home telephone number, especially if you are ex-directory, in case
we need to contact you for any reason. Knowing your mobile number, if you have one, would also be helpful.
