For patients that were registered with Dr E Hughes before 31^st March 2015

We have allocated all of Dr Hughes patients to the list of another GP in the practice.

We have divided the patient groups via Postcode

If your postcode falls between

PO31 8ED to PO33 1LN your allocated GP is DR H Elsmore  

PO33 1LQ to PO33 2YE your allocated GP is Dr Spencer Fox

PO33 2YF to PO33 3BG your allocated GP is Dr A Gove

PO33 3BY to PO33 3RG your allocated GP is DR J Hesse

PO33 3RY to PO37 6HP your allocated GP is Dr Sally Ridout

Your usual/or accountable GP

As a registered patient of this surgery Dr Hughes was recently listed as your registered GP.

Dr Hughes retired from this surgery on 31^st March 2015. We were unable to secure a replacement GP for him due to the national shortages of GPs. Therefore we have had to allocate all of Dr Hughes patients to the other partners in the surgery.  This will not affect the doctor you normally see or your preferred choice of GP this will remain the same.

You may see whoever you wish subject to availability of course. 

In the past The NHS authorities that look after patient registrations would inform all patients when their GP retires, this is no longer the case and it has fallen to practices to inform the patients.

The Government have introduced new measures to make sure that all registered patients have an accountable GP, and that they are informed of that accountable GP.

Please accept this as information regarding your Accountable GP.  This will not affect your healthcare in any way or the choice of GP you wish to see.
